Protection for Your Medical Records
In an effort to protect you and your personal information, Bayfront Medical Center asks patients who request copies of their medical record to sign a medical authorization form. This form ensures the safety of your information. Bayfront constantly strives to provide quality healthcare to all it serves, including safety practices that protects patients even after their stay.

Please Note: There will be a charge for records provided ($1.00 per page). Please allow 7-10 days for the records to be available. Records will be provided free of charge within 24 hours for treating physicians who are authorized to receive copies of patient records.
