We take everything into consideration to design an individualized plan to help you make the most of your abilities to achieve the highest level of independent living possible. Once admitted, your interdisciplinary rehabilitation team will work with you to determine your individual needs. Patients and physicians discuss and set realistic goals for recovery. Goals may include returning to work after an injury or re-establishing the quality of life experienced before the injury. The team shares goals with your loved ones and may be revised to reflect your progress and personal choices.

Bayfront offers outpatient medical rehabilitation at the Bayfront Family Health Center, conveniently located on the campus of Bayfront Medical Center. Specific goals are determined and a treatment plan is developed and implemented for each patient. This CARF-accredited facility offers the following services: physical therapy, occupational therapy, speech-language pathology, aquatic therapy, driver evaluation, wheelchair evaluation, case management and social services.
